Install the Chrome or Edge extension, or go to app.scribept.com from any browser. Log in with the credentials from your setup email, set your personal password, then re-login.
Name your encounter, tap + ADD NOTE TYPE to choose a format, then tap + ADD CONTEXT. At least one context is required — add a Recording, written comments, intake form info, or a past note.
Press RECORD to capture audio. Speak measurements, interventions, and observations aloud — the more detail you provide, the higher quality your note. Hit SUBMIT when done.
Review the NOTES tab, edit any section for accuracy, then click SAVE & PASTE TO EMR. Compatible with TheraOfficeNX, WebPT, Prompt, Jane, and QuickEMR.
ScribePT generates better notes when you give it richer context. Here's what separates a complete note from a vague one.
Say "initial evaluation," "progress note," or "discharge summary" so ScribePT uses the correct structure and tone.
Include the diagnosis or injury — e.g., "rotator cuff repair post-op week 4" — so the note reflects accurate clinical framing.
Say "left shoulder" or "right knee" explicitly. ScribePT cannot assume laterality — if you don't say it, it won't appear in the note.
Verbalize specific values as you assess — e.g., "hip flexion strength is 3+/5 with pain" or "shoulder flexion ROM is 110 degrees." ScribePT can hear what you say but cannot see what you do, so any finding you don't narrate will be missing from the note.
Narrate the treatments performed, exercises completed, and the patient's response rather than leaving it implied.
Attach prior notes or encounter summaries for progress notes and discharge summaries so ScribePT can track improvement.
"We did some exercises" gives ScribePT nothing to work with. Be specific about what was performed and why.
Without a condition or diagnosis, ScribePT cannot tailor the note structure to the clinical context.
Saying "the shoulder" instead of "the left shoulder" leaves laterality out of the note entirely.
If you measure strength or ROM without narrating the value aloud, it will not appear in the note. ScribePT hears your words — it cannot observe your hands.
One or two sentences rarely produce a complete note. ScribePT can only document what it hears.
Progress notes without previous context can't reflect improvement or compare to baseline — always use +ADD CONTEXT.

No worries — OTPs can take up to a minute to arrive. If you've waited and still don't see it, check your spam or junk folder, and make sure your inbox isn't blocking emails from ScribePT. It's also worth confirming your device and internet connection meet our minimum specs (Section 2.1 at scribept.com/support). If it's still not showing up, reach out to us using the Help button below and we'll get you sorted.
That error means the email you entered isn't associated with an account in our system. First, double-check for any typos — it's an easy one to miss! If the email is correct and you're still seeing this, use the Help button below and our team will look into it and help you get access.
A few things to check: the code is case-sensitive, so make sure it's entered exactly as it appears in your email. If you copied and pasted it, watch out for any extra spaces or stray characters at the beginning or end. Also, if you requested multiple codes, close and reopen the extension before entering the most recent one — older codes get invalidated.
This one usually comes down to two things. First, double-check that the OTP you entered is correct — it's case-sensitive and shouldn't have any extra spaces. Second, if your account has IP-based login restrictions, you may be trying to log in from an unrecognized network. Try switching to a recognized connection, or reach out to your account administrator to update the IP settings.
The extension should update automatically, but if you suspect it hasn't, here are two ways to force an update:
We recommend obtaining patient consent before using ScribePT, though the specific requirements vary by state and practice setting. Most of our users include a brief consent statement in their intake forms. Unfortunately, we're not able to provide consent forms or specific legal guidance, since requirements differ so much across locations and disciplines — but your compliance officer or professional association would be a great resource for that.
We understand it might seem convenient, but sharing login credentials isn't permitted. Each provider needs their own unique credentials — this is both a HIPAA requirement and part of ScribePT's Terms and Conditions. It also ensures that your documentation is accurately tied to you as the clinician. If you need to add a colleague to your account, use the Help button below and we'll get them set up.
Absolutely. We take security very seriously. ScribePT requires a unique username, password, and one-time password (OTP) every time you log in. All data — both at rest and in transit — is encrypted using healthcare-grade standards that align with HIPAA requirements.
Due to HIPAA compliance requirements, recordings can't be permanently deleted. However, you can archive any encounter and update your filters so that archived visits don't show up in your default view — keeping things clean without compromising compliance.
Yes! ScribePT supports a wide range of languages including Spanish, German, French, and Japanese — and it can even handle mixed-language conversations like Spanglish. One thing to note: your final note will always be generated in English, regardless of what language was spoken during the session.
Not at this time. ScribePT doesn't have a direct connection to your EMR, so it only works with the context you provide for each encounter. For progress notes or discharge summaries, we strongly recommend using the +ADD CONTEXT button to attach previous notes — that way ScribePT can compare and track progress accurately.
Yes! There are a few ways to use ScribePT on a mobile device:
Yes! That's what our Multi-Session feature is for. Under +ADD NOTE TYPE, select Multi-Session and record all your patients in one continuous audio file. ScribePT uses speaker recognition to separate each patient and generate individual notes. For best results, call each patient by name during their visit. Just keep things to 8 patients or fewer, and try to stay within the 1.5–2 hour recording limit.
It can! ScribePT is designed to handle background noise, but there are a few things that help: keep your device as close to your conversation as possible, and consider using dictation mode or written comments if ambient noise is especially heavy. A lot of our users in busy clinic settings have great results with a Bluetooth lapel mic as well.
You can adjust your microphone's sensitivity through your device's system settings. On Windows, go to Settings → System → Sound → Input. On a Mac, go to Settings → Sound → Input. Lowering the sensitivity will help reduce background noise pickup. If your notes are coming out incomplete, try bumping the sensitivity up instead. You can find detailed steps in Section 3.3 at scribept.com/support.
This almost always comes down to microphone permissions. Here's how to fix it:

ScribePT needs an internet connection when you start a recording, when you submit, and while your note is being processed. However, if you temporarily lose connectivity during an active recording, you can keep going — the recording won't be interrupted. Just make sure your connection is restored before you hit Submit.
The WiFi icon on your home screen reflects your current connection quality. Green means you're good to go. If it turns yellow or red, your connection is weak or unstable — we'd recommend waiting until it's back to green before submitting, since a poor connection at that stage could result in lost context.
Yes, definitely — especially for longer sessions. If your screen locks or your device goes to sleep mid-recording, it can interrupt your session. Before you start, go into your device settings and disable auto-sleep and screen timeout so you don't run into any issues.
Yes — we recommend keeping individual recordings to 1.5 to 2 hours or less. Longer recordings can affect processing reliability and note quality. If you're approaching that window, it's a good idea to go ahead and submit and start a fresh encounter rather than letting the recording run too long.
Think of ScribePT like a very attentive scribe — it can only document what it hears. The more specific and explicit you are, the better your note will be. Call out objective data points as you go: for example, say "Right knee flexion is 128 degrees, compared to 115 on the left" rather than something vague like "ROM is limited." A helpful reminder: ScribePT can hear what you're saying, but it can't see what you're doing — so narrate your assessments and interventions as you perform them.
ScribePT gives you several ways to provide context for your note: Audio Recording (ambient or dictated), Written Comments or scratch notes, Intake Form information, Past Follow-Up Notes, Past Evaluation Notes, and Referral Information. You'll need at least one type of context added before you can submit an encounter.

For microphones, we suggest starting with your device's built-in mic — ScribePT was designed with that in mind and it works well for most users. If you'd prefer an external mic, just make sure it's compatible with your device and has enough battery life for your sessions.
Your context is saved when you click Submit and see the confirmation message "Encounter Submitted Successfully!" Until you see that message, nothing has been saved — so make sure you always hit Submit before closing the app. If you're concerned about a recording that may have been lost, reach out via the Help button below as soon as possible, since recordings are typically recoverable for at least 24 hours.
In most cases, your note will be ready within 2 minutes of submitting. If it's taking longer than that, something may be off — go ahead and reach out to us using the Help button below and we'll look into it.
Not at all! After you hit Submit, you'll have the option to start another encounter right away. Just select "Submit another encounter" to go back to the home screen. When your note is ready, you can find it under the "My Encounters" button at the top of the tool.
We like to think of ScribePT as a high-achieving student — it's aiming to get you 80–90% of the way there. The quality of the output is directly tied to the quality of the context you provide. That's why we always recommend reviewing and finalizing your note in your EMR before signing off. ScribePT is here to do the heavy lifting — you bring the clinical expertise.
Start by regenerating the note — sometimes that's all it takes. If it's still missing information after regenerating, open the Context tab and check whether the information is actually in your transcript. If it's there but not showing up in the note, head to Personalize in the Custom Notes Workshop and add some cueing so ScribePT knows where to place that information in your template. If it's not in the transcript at all, that's a sign your microphone or recording environment may need a few adjustments.
What you're describing is sometimes called a hallucination, and it's typically caused by background conversations being picked up by your microphone. A few things that help: keep your recording device as close to your conversation as possible, try to record somewhere quieter, and if it's a recurring issue, consider requesting a USB lapel microphone through the Help button below — our team can help with that.
ScribePT doesn't automatically carry over information from previous visits — it only works with the context you provide for the current encounter. To include your initial eval, click the +ADD CONTEXT button and select the relevant information from your prior visits. This is especially important for progress notes and discharge summaries.
ScribePT intentionally does not learn from your individual note edits — and that's by design. Edits you make to one note are often specific to that patient or encounter, and automatically applying them to all future notes could actually cause problems. If you want to change how your notes are structured or written going forward, the best path is the Personalize feature in the Custom Notes Workshop.
If you're using a compatible EMR — TheraOfficeNX, WebPT, Prompt, Jane, or QuickEMR — the Paste to EMR feature handles it with one click. If your EMR isn't on that list, no worries! Use the Copy Text button above the note to copy the full note, or hover over any section header to copy just that section.
Paste to EMR lets you push your completed note directly into your EMR with a single click, using the ScribePT Chrome or Edge extension on a desktop or laptop. Before you click "Save & Paste to EMR," make sure the note field in your EMR is blank — the feature typically overwrites whatever is already there. Then just let it run; it can take up to 2 minutes to complete.
Unfortunately, no — Paste to EMR is only available through the Chrome or Edge browser extension on a desktop or laptop. If you're working in the web app, you can still get your note into your EMR by using the Copy Text button to copy the full note, or hovering over individual section headers to copy specific sections.
We recommend staying focused during the paste process — switching tabs, opening other apps, or closing the extension can interrupt it. Once you've clicked "Save & Paste," let it finish completely before moving on. You're welcome to review the note as it pastes, just avoid navigating away.
This is a known issue. Third-party browser extensions can sometimes conflict with the Paste to EMR process. If you're seeing paste failures, try disabling your other extensions one at a time to identify the culprit, or disable them all temporarily while using Paste to EMR.
You can, and sometimes that's helpful for quick adjustments. That said, we recommend treating ScribePT as the place to capture context, and making your final edits in your EMR — the EMR is the official record. Edits made in ScribePT before pasting can also occasionally cause issues with the Paste to EMR feature.
Context changes aren't applied automatically after submission. After editing anything in the Context tab, you need to click "Update Encounter" to save your changes and trigger a note regeneration. It's an easy step to miss, but once you click it you should see an updated note shortly.
In the Notes tab, look for the "+ Add Section / Information to Note" option — that's your go-to for expanding content within a section. You can also go back to the Context tab, add written comments with the additional details, and then click "Update Encounter" to regenerate the note with that new information included.
Notes should generate in under 2 minutes, so if things feel sluggish, a few common culprits are worth checking: your device or network may be below our recommended specs, you might be using a remote desktop (like LogMeIn), a VPN could be limiting throughput, antivirus software may be throttling the app, or you may have too many tabs open. Start by checking your specs, then work through the list. If you're still stuck, reach out via the Help button below.
We don't recommend it. Remote desktop environments like LogMeIn can cause a range of issues — login problems, failed note generation, and other unexpected behavior. For the best experience, use ScribePT directly on your local device.

That's a quick fix! In Google Chrome, click the three-dot menu in the top right corner → Appearance → Page Zoom, and reduce the percentage until the entire extension is visible. Most users find somewhere between 80–90% does the trick.
